Medicare Policy
Insurance coverage through Medicare (United States) is not accepted at our practice. This includes all Medicare plans and Medicare replacement or advantage plans.
HMO Referral Policy
If you are enrolled in an HMO insurance plan, a referral from your primary care provider is required prior to your appointment. It is the patient’s responsibility to request the referral from their primary care provider and to ensure that it has been sent and received by our office before the scheduled visit.
If a valid referral is not received prior to the appointment, the visit may need to be rescheduled or the patient may be responsible for the full cost of the visit according to office policy.
Laboratory Services
Our office works with several laboratory partners, including Labcorp, Medical Diagnostic Laboratories, and Quest Diagnostics for diagnostic testing.
For your convenience, a laboratory technician from Labcorp is available in our office for bloodwork collection only. If your testing requires services from MDL or Quest Diagnostics, or if your insurance requires a specific laboratory provider, you may be directed to an external laboratory location to complete the testing.
Patient Responsibility & Verification
Insurance networks and coverage participation may change over time. Insurance provider directories may list Dr. Higginbotham as in-network; however, this does not always reflect our office’s participation policies. Patients are encouraged to contact our office prior to scheduling to confirm that their specific insurance plan is accepted. If a plan is not accepted by our practice policy, services may be considered self-pay.
