Office policy
Welcome to the office of Chandra Higginbotham. Our goal is to provide high-quality, compassionate women’s healthcare in a respectful and efficient environment. The following policies are designed to help ensure smooth scheduling, timely care, and clear communication for all patients.
Payments
All co-pays and deductible are due prior to treatment.
If we are not a participating provider with your insurance company, full payment is required at the time of service. Additionally, if your insurance company does not cover the services provided, you are responsible for the full payment.
Patients are responsible for understanding their insurance coverage, benefits, and financial responsibilities. Verification of insurance acceptance prior to scheduling is strongly recommended.
Appointments
We offer a variety of appointment types to meet your healthcare needs, including:

Virtual (Telehealth) Consultations
Please arrive on time for your scheduled appointment. Late arrivals may require rescheduling to avoid delays for other patients.
Appointment Cancellations, Rescheduling & No-Shows
We understand that life can be unpredictable and appreciate your cooperation in keeping your scheduled appointments.

Cancellations or rescheduling must be made in advance, require 24 hours' notice.

Same-day cancellations or reschedules may result in a $50.00 fee if a valid reason is not provided.

No-show appointments (failing to attend a scheduled appointment without notice) may also result in a $50.00 fee.

Patients who miss more than three (3) or more unexcused appointments without proper notice may be asked to transfer care to another provider to ensure continuity of care for all patients.
This policy helps maintain timely access for all patients and ensures everyone receives the care they need.
Well-Woman Exam Reminder
A Well-Woman Exam is focused on preventive care and overall wellness.

WWE visits are not for sick or urgent concerns.

If you have new symptoms or health issues, please schedule a problem-focused visit.

Routine questions and prescriptions requests are handled during regular office hours . Emergencies are addressed as soon as possible. For urgent medical emergencies, please call 911.
Prescription Refill Policy

Refill requests should be submitted during appointments when possible.

Requests between visits may be submitted through the office or your pharmacy.

Please allow 48–72 business hours for processing.

Certain medications may require a follow-up visit before refills can be authorized.
Medical Records Request Policy
To send your medical records to another facility, our office requires a signed Release of Records form from the receiving facility. This ensures that all transfers comply with HIPAA privacy regulations.
Processing may take several business days depending on the volume of records requested, and records will only be released once proper authorization is received.

To reduce exposure to illness, especially for our obstetric patients, we ask that you PLEASE not bring children to the office. If bringing children is unavoidable, please ensure a caregiver is available to supervise then in the lobby.

Please do not eat or drink in the office to help us maintain a clean and professional environment.
Please do not rearrange the furniture.
